課程簡介
第一堂：Opportunistic Infections in critical ill-patients in ICU(余文良醫師)
Clinicians and other medical professionals fight a never-ending battle against a variety of newly developing infectious pathogens in the modern intensive care unit (ICU). Advances in medicine, including life-sustaining devices, immunosuppressive medications, and sophisticated surgical procedures, have unwittingly produced conditions that make patients prone to opportunistic infections. Serious infection is a real risk in these critical care environments, and the pathogens are frequently sneaky, adaptable, and incredibly hardy. This lecture describes emerging opportunistic infections in the ICUs, including cytomegalovirus enterocolitis (such as colitis and duodenitis), cytomegalovirus pneumonia, invasive pulmonary aspergillosis, and Pneumocystis jirovecii pneumonia. These contents serve as stepping stones towards a comprehensive understanding of each pathogen, helping us to devise more effective strategies for diagnosis and treatment.
第二堂：Management for MDR-GNB in ICU (賴志政醫師)
抗藥性菌已成全球重大公共衛生威脅，預估2050年每年恐致1000萬人死亡。台灣多重抗藥性菌（MDRO）在醫療院所顯著上升，COVID-19疫情更加劇其傳播與抗生素使用挑戰。新一代抗生素雖提供治療選項，但耐藥株迅速出現。推動抗生素管理與非抗生素策略（如感染管制、診斷工具）對減緩MDRO威脅至關重要。
第三堂：Management for Infections of Immunocompromised Hosts(蔡進相醫師)
免疫低下宿主因不同免疫缺陷類型面臨特定感染風險。中性球低下發燒為感染急症，致病菌以腸道桿菌科為主，治療需立即給予廣效抗生素，高風險患者須涵蓋綠膿桿菌。侵襲性黴菌感染常見於長期嚴重免疫低下、抗生素使用、全靜脈營養及免疫抑制患者，包括念珠菌、隱球菌、麴菌及白黴菌感染。移植患者伺機性感染亦為臨床一大挑戰。免疫低下宿主之感染，診斷需積極以檢驗、影像尋找病灶，治療應根據風險分層選擇適當抗生素。
第四堂：Managing emerging infectious diseases in the ICU(謝宗達醫師)
新興傳染病的威脅從未遠離，從少數高致命性的病毒出血熱，到引起全球的大流行呼吸道疾病如流感或COVID-19，都需要重症團隊在加護病房進行治療。重症團隊平時就得對傳染病重症有基本認識及相關照護訓練，並在需要時站在第一線挽救病人生命。
