CHALLENGE AND QUARANTINE HOTELS MANAGEMENT FOR THE COVID-19 PATIENTS (EXPERIENCE SHARING FROM A QUARANTINE HOTEL IN NEW TAIPEI CITY)

The COVID-19 outbreak starts with the China Airlines and the Novotel clusters before spreading to tea parlors in Wanhua recently. Although there are many hospitals in Taipei City and New Taipei City, many COVID positive patients still could not get into the hospitals for treatment. Many patients are requested to stay home waiting for admission. Some of the COVID -19 patients break the quarantine rules and cause more infections to the family members and other citizens.

Considering of setting the mobile cabin hospital could be late for the emergency situation, local governments set up the quarantine hotels for the COVID-19 patients. As the time was limited and we are lacking for the experience, we found a lot of problems in managing COVID-19 patients in our quarantine hotel. Some patients died in the other quarantine hotels, then some expertise suggested that the patients may need the pulse oximeters in quarantine for preventing silent hypoxia and sudden death. After setting up the oximeters in the quarantine hotels, the emergency situation got better.

Recently, journalist reported one COVID-19 patient escaped from the quarantine hotel, and we found a homeless patient took triple dinner dishes from the other rooms. Some patients complained that they could not get the cellphone charger and even the sanitary napkins in other quarantine hotels. Some other patients complained that they could not get the drugs for releasing the symptoms easily. And, some patients got chest discomfort in the quarantine hotels, but they cannot get the ECG immediately.

Many quarantine hotels were set up in a very short time. So, there were many problems need to be solved in a very limited time. We hope our experience could be a model for setting up more and better quarantine hotels in the near future.

新冠肺炎防疫旅館之挑戰與可能之防疫措施(以新北市某防疫旅館團隊為例的經驗分享)

新冠肺炎肆虐,每天增加數百位新確診病患,醫院隔離病房實在無法負荷。若將病患留置家中觀察則會擔心病患會違反規範外出,或感染家中成員等問題。因為輕症防疫醫院趕建不及,雙北市以及部分縣市改設置防疫旅館以安置病患。在無實戰經驗的情況下,發現不少問題。因為少數病患在防疫旅館觀察期間不幸往生,專家學者才發現不論是在防疫旅館或居家觀察,確實需要配置血氧濃度監測儀以監測病人血氧的變化。所幸各縣市衛生局緊急採購血氧濃度機器之後,危急轉診個案減少,杜絕了不幸事件的發生。

防疫旅館在營運作業中,除醫療需求之外,也有衍生很多管理及服務的問題。最近某防疫旅館發生確診病患逃跑事件,也有某防疫旅館發生遊民入住後,偷吃隔壁房間餐食的問題。部分女性病患也有採買生理期用品或是手機充電器等需求待解決。在醫療業務上,入住病患常常會有發燒,咳嗽或是嘔吐腹瀉等問題需要投藥、年老病患除給予氧氣及一般藥物外,也可能因為胸痛發作需要心電圖檢查等。但在醫院醫療資訊系統及護理資訊系統無法上線的情況下,是否給予成藥成為可討論的問題。

防疫旅館要做到完善,在開業前有很多設定需要解決。以下就針對防疫旅館的醫療組,飯店組及警衛組做詳盡的說明及建議。本文拋磚引玉,希望未來防疫旅館醫療照護更加成熟。防疫旅館團隊集合醫療團隊,市府警察局團隊以及飯店經營團隊共同運作。本文將依據疾管署訂定之「新冠肺炎防疫旅館設置管理指引」為基礎1,介紹三個團隊的工作內容,以供大家參考。
